To explore the features of circulating B cells, including their surface receptors, and measure serum levels of B-cell activating factor of the TNF family (BAFF) and proliferation-inducing ligand (APRIL), in patients with antineutrophil cytoplasmic antibody-associated vasculitis (AAV). For this investigation, blood samples were obtained from a cohort of 24 patients with active AAV (a-AAV), 13 patients with inactive AAV (i-AAV), and 19 healthy controls (HC). Serum BAFF, APRIL, IL-4, and IL-6 levels, along with the proportion of plasmablasts (PB) and plasma cells (PC), were markedly higher in the a-AAV group than in the HC group. In i-AAV, serum levels of BAFF, APRIL, and IL-4 were higher compared to those in the HC group. Memory B cells in the a-AAV and i-AAV groups showed reduced BAFF-R expression, while CD19+ cells, immature B cells, and PB/PC displayed elevated TACI expression in contrast to the HC group. Serum APRIL levels and BAFF-R expression in a-AAV exhibited a positive correlation with the number of memory B cells.
